Health Care & Prescription Drug Insurance

Point of Service Plan (POS) – similar to an HMO. Can receive care from in-network or out-of-network providers without a referral from your Primary Care Physician.

Health Maintenance Organization (HMO) – HMO requires use of in-network providers for scheduled services. Emergency/Urgent care is covered anywhere.

HMO/Health Savings Account – Same as HMO above with $1300/2600 single/family deductible. Tax-free bank account. Employer contributions.

   Dental Insurance

Maximum benefit – $2000/person/year 

Cleaning & X-Rays – 100%

Fillings – 80%; Extractions, crowns, bridges & dentures – 50% – Subject to deductible

$1500 orthodontic benefit/lifetime

  Life Insurance 2 x Annual salary to a maximum of $300,000
  Accidental Death and Disability 2 x Annual salary to a maximum of $300,000
   Short Term Disability 60% of weekly wage to maximum of $1500
   Long Term Disability 60% of monthly wage to maximum of $10,000
   Voluntary Vision Exam & Lenses (12 mths) – Frames (24 mths)
   401(k) No waiting period, immediate entry, pre-tax or Roth, Match – 100% up to 3% and 50% to 5% (total 4% match on a 5% employee contribution), vesting 100% immediately
  Employee Assistance Program Confidential counseling sessions. Each employee/family member is entitled up to 5 free counseling sessions per problem.
  Paid Time Off and Holidays  Shine offers a minimum of 80 PTO hours annually with 8 paid holidays
 SHINE-Health-Parential_Leave-Icon Paid Parental Leave Shine offers up to 6 weeks of paid parental leave following the birth/adoption/foster care placement of a child
