Health Care & Prescription Drug Insurance

Health Maintenance Organization (HMO) – HMO requires use of in-network providers for scheduled services. Emergency/Urgent care is covered anywhere.

Point of Service Plan (POS) – similar to an HMO. Can receive care from in-network or out-of-network providers without a referral from your Primary Care Physician.

Preferred Provider Organization (PPO) – Nationwide coverage. Can receive care from in-network or out-of-network providers without a referral from your Primary Care Physician. Must live outside of network service area to elect this plan.

High Deductible (HSA) versions of each of the plans are available as well. HSA HMO & POS In-Network deductible is $1350 single/$2700 family. HSA POS Out-of-Network & PPO deductible are $2000 single/$4000 family.

  Cash-In-Lieu Opt Out Credit Employees who waive medical insurance are eligible for a $1,500 annual credit paid in semi-monthly increments.
  Flexible Spending Accounts (FSA)

Full Flex – Available to those that do NOT have an HSA Health Plan – Can be used for medical, dental & vision expenses.

Limited Flex – For those on an HSA Health Plan – Can be ONLY be used for dental & vision expenses.

Dependent Care Flex – For care expenses of children under 13 and elderly dependents.

   Dental Insurance Maximum benefit – $2000/person/year 
Cleaning & X-Rays – 100%
Fillings – 80%; Extractions, crowns, bridges & dentures – 50% – Subject to deductible
$1500 orthodontic benefit/lifetime
   Voluntary Vision Exam & Lenses (12 mths) – Frames (24 mths)
  Life Insurance 2 x Annual salary to a maximum of $300,000
  Accidental Death and Disability 2 x Annual salary to a maximum of $300,000
  Voluntary Supplement Life Employee – Up to $500,000
Spouse – Up to $50,000
Children – Up to $10,000
Guaranteed issue up to $100,000 employee/$10,000 spouse for new hires. Children always guaranteed.
   Short Term Disability 60% of weekly wage to maximum of $1500
   Long Term Disability 60% of monthly wage to maximum of $10,000
  Voluntary Accident Insurance Benefit paid directly to employee based upon various medical services received due to an accident.
   401(k) No waiting period, immediate entry, pre-tax or Roth, Match – 100% up to 3% and 50% to 5% (total 4% match on a 5% employee contribution), vesting 100% immediately
  Employee Assistance Program Confidential counseling sessions. Each employee/family member is entitled up to 5 free counseling sessions per problem. Additional services include financial consulting, legal services, and various other online resources available.
  Paid Time Off and Holidays  Shine offers a minimum of 80 PTO hours annually with 8 paid holidays
 SHINE-Health-Parential_Leave-Icon Paid Parental Leave Shine offers up to 6 weeks of paid parental leave following the birth/adoption/foster care placement of a child
