|Health Care & Prescription Drug Insurance||Health Maintenance Organization (HMO) – HMO requires use of in-network providers for scheduled services. Emergency/Urgent care is covered anywhere.
Point of Service Plan (POS) – similar to an HMO. Can receive care from in-network or out-of-network providers without a referral from your Primary Care Physician.
Preferred Provider Organization (PPO) – Nationwide coverage. Can receive care from in-network or out-of-network providers without a referral from your Primary Care Physician. Must live outside of network service area to elect this plan.
High Deductible (HSA) versions of each of the plans are available as well. HSA HMO & POS In-Network deductible is $1350 single/$2700 family. HSA POS Out-of-Network & PPO deductible are $2000 single/$4000 family.

|Employee Assistance Program||Confidential counseling sessions. Each employee/family member is entitled up to 5 free counseling sessions per problem. Additional services include financial consulting, legal services, and various other online resources available.|
